Five Boss Trophies for one boss-specific Unique sounds fair until you burn a whole night on Torment and get the wrong roll anyway. That's Season 13 Diablo 4 in a nutshell. If you're building around Diablo 4 Items and target farming instead of praying at random chests, this Diablo 4 Boss Trophy guide is the stuff you need first. The short version: trophies drop in Torment, the Horadric Cube turns 5 matching trophies into a Unique from that boss pool, and Greater Affixes can show up on the result.
You won't even see the full system until you finish the Lord of Hatred campaign and unlock the Horadric Cube with Temis in Skovos. From there, open Item Transmutation, pick Amalgamation, and feed it 5 of the same Boss Trophy. I tested this after the Season of Reckoning changes and, not gonna lie, it feels way less messy than old summon-mat farming. Still RNG, sure. But at least you're rolling inside one boss's loot pool instead of tossing gold into the wind.
Here's the thing though: the trophy source matters more than the boss kill sometimes. Abhorrent Heart comes from the Harbinger of Hatred or World Bosses. Distilled Fear drops from The Beast in the Ice, Nightmare Dungeons, Mercenary Bartering, and Tree of Whispers. Exquisite Blood is tied to Lord Zir, World Bosses, and Legion Events, while Fresh Meat comes from The Butcher, World Bosses, and Legion Events. Living Steel still points you toward Grigoire and Helltide chests, Malignant Heart comes from Echo of Varshan, Pincushioned Doll from Andariel, Shard of Agony from Duriel, and Judicator's Mask from Urivar.
I'd farm Tree of Whispers when I'm half-awake, then swap to Helltide or Nightmare Dungeons when I actually care about pace. World Bosses are nice bonus stops, not a full plan. Your mileage may vary, but chasing one trophy source at a time has saved me from that awful “I played three hours and progressed nothing” feeling.
Echo of Varshan is a good early target because Shard of Verathiel can fit several classes, and the side drops aren't trash either: 100,000 Steps for Barbarians, Vox Omnium for Sorcerers, stuff like that. Grigoire asks you to dance around lightning pylons, then throws at you Paingorger's Gauntlets, Hunter's Zenith, and The Mortacrux. Lord Zir is still blood puddle nonsense, but X'Fal's Corroded Signet, The Umbracrux, and Ring of the Midday Hunt make him worth learning. The Beast in the Ice can hand you Signet of Pelghain, and Lair Butcher has The Butcher's Cleaver plus Staff of Zerae, which is funny because he still jumps me at the worst possible time.
For Mythic Uniques, Duriel, Andariel, Belial, and the Harbinger of Hatred are where the sweat starts. Duriel can drop Razorplate and Rod of Kepeleke, while Andariel has Godslayer Crown and Ebonpiercer in her pool. Belial is the spicy one: a lesser Belial can invade after other Lair Bosses, drops Betrayer's Husks, and 2 of those open the real fight. Beat him and you can pick another Lair Boss loot table, with Ancestral Uniques and Greater Affixes on the menu. That tracks with Blizzard Diablo 4 bug fixes and Diablo 4 hotfix 5 notes, since Greater Affix rates had to behave for Amalgamation to feel sane.
Bartuc is the farm I didn't expect to enjoy. In Infernal Hordes, you need at least 666 Burning Aether, usually by picking Aether Fiends, Chaos Waves, or Soulspires, and then he can cough up Arreat's Bearing or Raiment of the Infinite. The Cow King puzzle is sillier but real: grab the Bloody Wooden Shard, Intricate Metallic Fragment, and Musty Tome, open the Forlorn Hovel, get the Stamina Potion, then Cube your way to Cow Island for Cow King's Crown. Mephisto still has fuzzy numbers on Mythic Seal drop rates, so take any “best Torment tier” claim with a grain of salt; if you're short on time and comparing outside help, U4GM is the kind of marketplace players check for game currency or item services, but the best farm is still knowing exactly which boss you're feeding trophies into.
